Here are the restaurants and other food facilities that San Bernardino County health inspectors temporarily shut down because of imminent health hazards between Dec. 11 and 17, 2020.

Jenny’s Restaurant Pupeseria, 741 W. Holt Blvd., Ontario



Closed: Dec. 11

Grade: 65 (no letter grade)

Reason: Unsanitary conditions. San Bernardino County closes restaurants for scoring below 70. The inspector visited in response to a complaint about conditions in the kitchen and found four critical violations. There were two live roaches near a food-prep table, fresh and old rodent droppings in a cabinet and about 20 flies in the kitchen, one of which landed on a container of cheese. Multiple containers of refrigerated food were at unsafe temperatures. They were in a refrigerator that also contained some moldy fried fish. The restaurant wasn’t keeping track of how long it was leaving food out at room temperature. Among the 13 lesser violations, the walk-in cooler door wouldn’t close properly, frozen meat was being thawed in a bucket on the floor under a sink, and the stove, grill, refrigerators, other equipment, walls and shelving were excessively dirty. This restaurant has received a B or lower on all five graded inspections in the past two years.

Reopened: Dec. 14 after a reinspection verified the facility had been cleaned and sanitized. It still must undergo a rescore inspection within 30 days.


Birrieria Xolos, 105 N. Euclid Ave., Ontario



Closed: Dec. 11

Grade: 91/A

Reason: Rodent infestation. The inspector found fresh rodent droppings on a prep table and near to-go containers. Among other violations, a reach-in refrigerator wasn’t keeping cold.

Reopened: Dec. 14 after the restaurant received pest-control service

Non-closure inspections of note

Here are selected inspections at facilities that weren’t closed but had other significant issues.

Yoshi, at 14177 Kentwood Blvd. in Victorville, was inspected Dec. 15 and received a grade of 83/B with two critical violations. The chute in the ice machine had black mold-like residue, with condensation dripping over it into the ice, which had some black pieces in it. Also, a wiping cloth wasn’t sanitized between uses. Among the six lesser violations, there were dry rodent droppings near a sink and the water heater. This was the restaurant’s third B or C grade since 2018

Zaky Mediterranean Grill, at 4323 E. Mills Circle Suite 102 in Ontario, was inspected Dec. 15 and received a grade of 84/B with two critical violations. There were five dead cockroaches, including one at the corner of the cook-line grill. And an employee didn’t wash hands before handling food. A follow-up was planned to make sure there wasn’t an ongoing roach problem. This was the restaurant’s second consecutive B grade.

Auraganic, at 14673 Ramona Ave. in Chino, was inspected Dec. 11 and received a grade of 80/B with two critical violations. A display cooler (which an employee said had been repaired two months earlier) was 75 degrees inside and 15 containers of food were at unsafe temperatures. And there was a moldy cherry tomato in another refrigerator. Among the seven lesser violations, there was a black mold-like substance in the ice machine, and the facility was operating outside the scope of its permit by selling soup that had been made in an unapproved slow cooker.

La Sexy Michelada, at 18150 Arrow Blvd. Suite I in Fontana, was inspected Dec. 10 and received a grade of 81/B with two critical violations. Two employees didn’t wash hands — one after moving a trash can. And 10 gallons of menudo in the refrigerator since the day before still hadn’t cooled down to a safe temperature. Among the six lesser violations, some shrimp and pico de gallo were at unsafe temperatures in two other refrigerators; one had a door that wouldn’t shut so cans had been stacked up to keep it from opening.

Updates from past weeks

The Dollar Tree at 980 N. Mountain Ave. in Ontario, which was closed Dec. 9 because of a rodent infestation, was permitted to reopen Dec. 10.

All food facilities in the county are routinely inspected to ensure they meet health codes. A facility loses four points for each critical violation and one to three points for minor violations. An A grade (90 to 100 points) is considered “generally superior,” a B grade (80 to 89) is “generally acceptable” and a C grade (70 to 79) is “generally unacceptable” and requires a follow-up inspection. A facility will be temporarily closed if it scores below 70 or has a critical violation that can’t be corrected immediately.
